Difficulty adjustment

Difficulty retargets every 60 blocks.

Each adjustment changes the leading-zero requirement by at most 1 bit. The representation is integral bits, so work at most doubles or halves per interval.

Fork choice uses cumulative work (totalWork in nvc_getConsensusInfo), not block height alone.
